Provide your comments and feedback from your appointment. Fill in your personal information, as well as the access code and your ticket number(s) given at your appointment [only the first and last are needed in a string of tickets] **you can find your access code and ticket numbers in the last line of the comments section on the reverse side of this form. 4.) Click the "Submit" button upon completion of all requested information, and then you may print the confirmation page which appears for your records 5.) If you have any difficulties, receive an error message, or would like us to register tickets for you, please call our office at the number on the reverse side of this form.
DOCTOR FLUE'S ARUBA TRIP SWEEPSTAKES RULES
Every physical, on site, appointment held with Doctor Flue, inc. receives a single numbered ticket. Additional numbered tickets are obtained by the ratio of 1 ticket received for every $100.00 increments worth of purchases spent with Doctor Flue, inc. The owner(s) of the ticket(s) are solely responsible to register any/ all numbered tickets on our company website: www.drflue.com within 24 hours of receiving the ticket(s).
Prize winner will receive one week lodging (from Sunday to Sunday) at Playalinda Resort in Aruba, which includes roundtrip (coach) airfare for two people, and taxi fare to and from Queen Beatrix Airport upon arrival and departure. The room tax (only) is also included. The Room is a studio, which can accommodate up to four adults. All other charges are not included. Any room charges incurred on the room account, other than the room tax, will not be paid by Doctor Flue, inc. All food, beverages, and entertainment are not included. The week of stay will be solely the discretion of Doctor Flue, inc., as this is subject to room availability and air transportation availability. Departure city is intended to be Detroit, Michigan. Intended time of stay will be one of the following: Week #9, Week #10, or Week #11 of the calendar year, according to "Playalinda" yearly timeshare schedule. Dates of room reservation and airport departure may change without prior notice. Valid passport is required for travel for each person.
Any changes in schedule after booking, or names on airline tickets, may be applied for, but may not be accommodated. Any expenses incurred because of any requested changes must be paid in full by the winning ticket holder prior to the change in application. Changes may not be able to be made. Doctor Flue, inc. is not responsible for availability of travel change procedure, or the success of such applications for change.
Drawing for the winner will take place on April 1st of the upcoming year. Odds of winning depend on the number of entries acquired within the time the sweepstakes is open. All federal, local and state laws apply. Void where prohibited by law. Both winning attendees of the Doctor Flue, inc. Aruba Trip Sweepstakes must be 18 years of age or older, or have at least one parent/ legal guardian. Winner will be determined by a random drawing conducted at Doctor Flue, inc. with a physical address of 1610 Dinius Rd., Tecumseh, Michigan 49286. Winner must produce the original winning ticket. There are no exceptions. If original ticket cannot be presented immediately, another subsequent drawing will be held to determine an alternate winning entry. Same rules apply to any subsequent winning selections. Drawing is not open to employees of Doctor Flue, inc, its affiliates, or members of its family. The winner will be notified by phone, and must submit an advertising/publicity/liability release within three days. The name of the sweepstakes winner will be published on our company website: www.drflue.com .
No purchase necessary. To enter DOCTOR FLUE, INC'S FREE ARUBA TRIP SWEEPSTAKES without a physical appointment on site, applicant must submit in their own handwriting a written request for a free estimate for his/her own chimney needs at his/her own property. Each request must be accompanied by photographs of the chimney needing service, or the area where the new chimney is to be installed. The written request, photographs, and a self addressed/stamped business size #10 envelope must be mailed to : DOCTOR FLUE ARUBA SWEEPSTAKES, 1610 Dinius Rd., Tecumseh, Michigan 49286.
Doctor Flue, inc. reserves the right to request actual measurements of the job estimate requested, and additional photos as necessary to complete the free estimate. One estimate per chimney is allowed one ticket per address only. Doctor Flue, inc. will then submit to the applicant a FREE ESTIMATE for applicant's request, and one (1) numbered entry ticket for the sweepstakes, in the self addressed/stamped envelope provided by the applicant. Upon receipt of the estimate and entry ticket the applicant must register his/her ticket at www.drflue.com . All rules stated apply.
All tickets must be registered with www.drflue.com by the day proceeding the drawing held on April 1st of the upcoming year, 5:00pm Eastern Standard Time. Doctor Flue, inc. does not assume responsibility for lost, misdirected, or late mail. Doctor Flue, inc. does not assume responsibility for server downtime, faulty internet connection, or circumstances out of our control. All correspondence, including addressing, must be in applicant's own personal handwriting. No machined and/or electronic correspondence shall be accepted.
